Home Health LVN Clinical Coordinator

Location: Dallas, TXJob Summary:TheraCare Home Health is seeking a detail-oriented and proactive Licensed Vocational Nurse (LVN) Clinical Coordinator to support clinical operations in an in-office setting. In this role, you will ensure critical clinical tasks are tracked, communicated, and resolved efficiently to support high-quality patient care. You will work closely with the Director of Clinical Services (DCS) and clinical team to manage workflows, monitor documentation, and facilitate timely follow-up on labs, physician orders, and patient-related communications. This role is essential in maintaining compliance, improving efficiency, and ensuring nothing falls through the cracks in the home health process.Qualifications:Current, unrestricted Licensed Vocational Nurse (LVN) licenseMinimum of 2 years of home health experience requiredStrong knowledge of home health documentation, physician orders, and regulatory requirementsExperience with Homecare Homebase (HCHB) requiredFamiliarity with Forcura, Mosaic, or similar document management systemsAbility to manage multiple tasks, follow-ups, and priorities in a fast-paced environmentStrong attention to detail and organizational skillsExcellent communication skills with clinicians, physicians, and leadership teamsAbility to work independently while supporting a collaborative clinical environmentEssential Functions:Track and follow up on lab results, diagnostic reports, and clinical findings to ensure timely resolutionIdentify abnormal or overdue labs and escalate concerns to the Director of Clinical Services (DCS) or appropriate clinicianEnsure lab results and updates are communicated to physicians and documented accuratelyMonitor patient charts to confirm all follow-up actions are completed and compliantAnswer and triage clinical phone calls from field clinicians, patients, and caregiversRoute urgent or complex clinical issues to RN leadership or DCS per protocolDocument all clinical communications and ensure issues are resolved efficientlyMonitor and manage HCHB task queues and Forcura/Mosaic workflows dailyTrack outstanding items including physician orders, Face-to-Face (F2F) documentation, medical records requests, and unsigned documentsProactively follow up with clinicians, intake, QA, and physicians to prevent delays in care or documentationReview Face-to-Face documentation for completeness and accuracyAssist with preparation and tracking of physician orders from initiation through signature and uploadCoordinate corrections, clarifications, and follow-up actions related to clinical documentationSupport the Director of Clinical Services with clinical tracking, workflow oversight, and process improvementIdentify workflow gaps, delays, and trends, and communicate findings to leadershipAssist with chart preparation for audits, surveys, and quality assurance reviewsPromote efficient workflows, compliance, and high standards of patient careWe are an Equal opportunity employer; We offer an excellent benefit plan to include 401K with match, CEU reimbursement, vacation, sick, holidays, medical, dental, and supplemental insurance Plans as well as a Highly competitive compensation package.Diversity, Equity, and Inclusion are at the heart of Cantex.
