- Perform comprehensive assessment of high-risk members.
- Collaborate with primary care providers to implement individualized care plans.
- Address and continuously reassess appropriate levels of care.
- Coordinate care transitions with healthcare professionals.
- Keep detailed records of clinical, functional, and fiscal outcomes.
- Identify opportunities for health promotion and illness prevention.
- Shift Requirements: Monday thru Friday, 8 AM - 5 PM.
- On-Site Work Location.
- License Requirements: NC, Compact License accepted.
- Associates or Bachelor's Degree in Nursing or related field.
- Current, unrestricted state Registered Nurse license.
- Case Management certification.
- Benefits are available to full-time employees after 90 days of employment.
- A 401(k) with company match is available after 1 year of service.
