CLINICAL INTAKE MANAGER- FULL TIME

divh2Clinical Intake Case Manager/h2pThe Clinical Intake Case Manager will be responsible for front line clinical assessment with the purpose of increasing census by helping determine eligibility of patients and facilitating their placement in the appropriate level of care. Must work in accordance with all applicable laws, regulations, and Life Care standards./ph3Essential Functions/h3ulliMust have the necessary skills to perform comprehensive nursing reviews for all incoming referrals and assesses for clinical appropriateness, regulatory compliance, and agency resources to meet the needs of the patient./liliMust establish and maintain productive working relationships with providers, referral sources, physicians, health care facilities and the community./liliMust be able to discuss cases with physicians and clarify physician orders./liliMust exhibit exceptional customer service and a positive behavior towards residents and/or patients, colleagues, physicians and community./liliAbility to education physicians, case managers, and insurance companies on facility specific programs encompassing the entire continuum of services./liliCommunicates with Administrator and Interdisciplinary team on a regular basis regarding resident and/or patients./liliResponsible for coordinating all inpatient admissions./liliManage coordination of the referral-to-admission conversion process/liliEnsures referrals are entered timely, accurately, and are completed prior to patient admission/liliRecognize barriers to admission respond appropriately and follow up on admissions variables./liliCoordinate all aspects of care with all referral sources to provide patients with a seamless transition to post-acute care setting and promote high quality care./liliServes as the primary point of contact between potential participants, and their families, the Catholic Health Services team./liliCommunicate all pertinent information to appropriate personnel and requesting, reviewing and uploading discharge documents in order to ensure clinicians have the current pertinent information necessary to provide excellent patient care./liliEnsures appropriate communication is maintained regarding changes to patient status in the scheduling of admissions./liliEnsuring the effective communication between Centralized Intake staff and Care Coordinators, referring physicians, board and care facilities, skilled nursing facilities, patients, families, hospice branch staff, patient care liaisons, other referral sources, and the organization./liliKeeps Central Admissions appraised of changes in providers request, change in patient status and unusual circumstances requiring further investigation prior to acceptance for services./liliEnsures appropriate communication is maintained regarding changes to patient status in the scheduling of admissions./liliDemonstrates flexibility in scheduling and readily accepts new admissions and emergency assignments as requested,/liliAdhere to departmental goals, objectives, standards of performance, and policies and procedures/li/ulp* Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of the position./ph3Other Duties/h3ulliAttend weekly/monthly meetings as required/liliPromotes positive employee engagement, teamwork, and mutual respect and safety work practices./liliMaintain your required certifications and mandatory skill updates./liliComply with all policies, local, state and federal laws and regulations./liliPerform other duties as assigned/li/ulh3Supervisory Responsibility/h3ulliMay serve as an interim department leader depending on need/li/ulh3Physical Requirements/h3ulliMust be able to lift and/or move up to 50 pounds and push/pull up to 250+ pounds, walk, climb stair or ladders, stand on feet for extended periods of time, etc./li/ulpThe job description is not designed to cover or contain a comprehensive listing of activities duties or responsibilities that are required of the employee.
