Care will be provided in the patient’s home or current placement (rehab center or hospital).About the CaseThis is a long-term, 24-hour case requiring consistent caregivers who can provide safe, dependable, and high-quality personal care. Candidates must be comfortable working independently while following a structured care plan.ResponsibilitiesAssist with all Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, feeding, and mobilityProvide care for a fall-risk patient with close attention to safetyAssist with transfers, repositioning, and mobilityProvide companionship and emotional supportDrive patient to appointments and run errands as neededFollow the patient’s individualized care planMaintain patient privacy, dignity, and comfort at all timesCommunicate clearly with the care team regarding any changes or concernsRequirementsValid HHA Certificate or CNA License (or ability to pass HHA competency test)Experience with Dementia and Alzheimer’s patients is requiredValid CPR CertificationNegative TB skin test or current chest X-rayLevel 2 Background ClearanceReliable transportationValid Driver’s License with active auto insuranceAbility to work independently and follow care instructionsShift Options8:00 AM – 8:00 PM (Day Shift)8:00 PM – 8:00 AM (Night Shift)Applicants may choose one shift or be considered for both.Travel RequirementsSome travel may be required depending on patient location.
