We are hiring a Speech Language Pathologist (SLP) for a Home Health position providing skilled care to patients in their homes in the Glen Ellyn and surrounding areas. This position is eligible for a $11,250 total bonus potential, including a $7,500 first year success bonus to reward the accomplishment of key achievements during your first year as well as an additional $3,750 sign-on bonus paid within the first 18 months of employment! To receive these bonuses, you must be in good standing & meet performance expectations, and in the role and status for which you were hired!

What You’ll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform comprehensive patient evaluations assessing speech, language, cognitive-communication, and swallowing function, developing individualized treatment plans to support safety, independence, and quality of life
• Educate and empower patients and caregivers on therapeutic exercises
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation

• Work independently, but never alone
• Travel to patient homes within assigned territory

SLP Qualifications

• Active Speech Language Pathologist, SLP license in the state you work
• One year of experience as a Speech Language Pathologist and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
