Home Health Aide (HHA) – Resident Transport & Support Services Split Rock Rehabilitation & Health Care Center is seeking compassionate and dependable Home Health Aides (HHAs) to assist residents with transportation and non-clinical support services throughout the facility. This role is ideal for individuals who enjoy working with seniors and helping residents maintain their comfort, dignity, and quality of life. Responsibilities Safely transport residents to and from therapy, activities, dining rooms, appointments, and other areas of the facility. Assist residents with wheelchairs, stretchers, and other mobility equipment. Provide support with resident comfort and non-clinical needs during transport. Escort residents to activities and appointments as directed. Observe and report any changes in resident condition or concerns to nursing staff. Maintain a courteous, professional, and resident-centered approach at all times. Follow facility policies regarding safety, infection control, and resident rights. Assist with keeping transportation equipment clean and in good working order. Support nursing and interdisciplinary teams as needed. Qualifications Current New York State Home Health Aide (HHA) certification required. Previous experience in a nursing home, rehabilitation center, or healthcare setting preferred. Ability to safely transport and assist residents with mobility needs. Strong communication and interpersonal skills. Reliable, compassionate, and team-oriented. Ability to work flexible schedules, including weekends and holidays as needed.
