Home Health Physical Therapist

A Home Health Physical Therapist delivers rehabilitation services directly in patients' homes, helping individuals regain strength, mobility, and independence after illness, injury, surgery, or due to chronic conditions. This role emphasizes individualized care in real-world living environments and requires strong clinical skills, independence, and flexibility.

Primary Duties:

• Perform in-home physical therapy evaluations and reassessments
• Develop and implement personalized treatment plans based on patient needs and home environment
• Provide hands-on therapy, therapeutic exercise programs, gait and balance training, and functional mobility work
• Assess safety risks in the home and recommend adaptive equipment or modifications
• Educate patients and caregivers on safe movement, recovery strategies, and home exercise programs
• Track patient progress and adjust care plans accordingly
• Maintain timely, accurate documentation in compliance with clinical and regulatory standards
• Collaborate with physicians, nurses, occupational therapists, and other care team members
• Travel to patient residences within assigned service areas

Qualifications:

• Doctor of Physical Therapy (DPT) or equivalent degree
• Active Physical Therapist license in the state of practice
• Current CPR certification (or ability to obtain)
• Valid driver's license and dependable transportation
• Prior home health experience preferred but not required

Physical Requirements:

• Ability to assist patients with transfers, walking, and mobility tasks
• Frequent standing, bending, and light-to-moderate lifting
• Ability to safely navigate various home settings
