Case Manager, Early Intervention Program - Physician Practice

Case Manager, Early Intervention Program Provides clinical care coordination in conjunction with healthcare providers and other support staff. Promotes prevention and health maintenance in an ambulatory care environment. Performs psycho-social assessment of the EIP patients and their families to determine and assist in resolving social, emotional and economic problems. Age/Patient Population served: Adolescent (13–17 years), Adult (18–64 years), Geriartric (65+ years). Education, Knowledge, Skills and Abilities Required Bachelor's degree in related field and/or four years clinical experience with individuals with HIV infection in either the acute care or ambulatory care setting Must be able to manage difficult or emotional customer situations; should be able to focus on solving conflicts and problem resolution Education, Knowledge, Skills and Abilities Preferred English/Spanish capabilities #J-18808-Ljbffr Hackensack Meridian Health Inc.
