Your responsibilities include the following :Conducting Diabetes Care and Education (DCES) participant assessments using the AADE7 framewor kDeveloping measurable goals, teaching plans, and individualized learning objective sProviding education using interactive and skill‑based methods to increase engagement and understandin gDocumenting patient assessments, learning plans, progress, and evaluations outcome sPerforming follow‑up assessments and track participant progress toward defined goal sReferring patients to appropriate health providers as neede dCommunicating relevant findings and updates to the primary care provider and healthcare tea mParticipating in developing program and teaching material sSupporting program accreditation requirements and participate in accreditation survey sEngaging in continuous quality improvement initiatives for Diabetes Care and Education Specialist (DCES )Maintaining required annual continuing education specific to diabetes and behavioral change strategie sStaying current with advancements in diabetes management, technology and educational best practice Required Qualification s:Active RN Licen seAt least 1 year experience with DCES progra msAt least 1 year experience developing therapeutic relationships and motivating participan tsStrong understanding of diabetes treatment and educati onAbility to present education conte ntStrong communication and teaching skil Preferred Qualificatio ns:Certified Diabetes Educator (CDE) or experience working in diabetes education prefer redAt least 3 years experience with DCES progr amsAt least 3 years experience developing therapeutic relationships and motivating participa #J-18808-Ljbffr
