SCHEDULE: Monday - Friday, 8:00am - 4:30pm; On-Call Rotation

The Nurse Practitioner (NP) provides health care to patients and their families through the application of advanced knowledge and skills. In collaboration with the Medical Director, the NP provides consultation in palliative care, symptom management and supportive care to meet the needs of the patient. The NP works collaboratively within the Interdisciplinary Group (IDG) and acts as resource person for staff and others on care related issues.

Qualifications and attributes include a Master of Science in Nursing, minimum one year as NP, and five years as RN. Preferred experience in long-term care, oncology, or hospice. Active RN license in state of practice and current certification as an NP required. Certificate as Hospice and Palliative Nurse preferred. Ability to create positive impressions and communicate with a variety of people. Maintain effective communication with patients, families, physicians and co-workers. Major areas of responsibility include performing patient visits and patient recertification activities such as the face to face encounter in coordination/collaboration with the IDG and/or Medical Director. Actively participating in IDG. Sharing, presenting and recording activities with other members and directing patient's plan of care as needed. Ensuring that documentation is compliant with organizational and governmental standards. Working collaboratively with staff, referring physicians, attending physicians, medical directors, and the caregiving team concerning care of patients and potential referrals. Assisting in the review of patient clinical profiles including medication and care plan review to assess appropriateness and make adjustments as needed. Providing cover for the physician when they are absent, to include answering patient calls, medication refill requests and other matters related to patient care. Educating caregivers, co-workers and the community on all aspects of palliative care and services provided by agency. Participating in developing current procedures regarding the provision of clinical care. Attending and participating in internal team meetings as needed. Preparing and maintaining accurate patient records, charts and documents to support the medical practice and reimbursement for services provided. Prescribing and ordering treatments and diagnostic tests according to licensure/credentialing body. Assisting in development of clinical practice guidelines I standards in support of quality palliative care as requested by supervisor. Participating in Quality Assurance Performance Improvement (QAPI) activities. Educating staff and patient/family on physical, emotional and psychosocial aspects of care, the disease process, symptom and pain management, signs and symptoms of death. Demonstrating expertise in pain management and symptom control as related to terminally ill patients. Accountable for proper utilization of hospice resources. Assisting with occasional nursing visits, administrative on-call duties, referrals and admissions as needed. Accountable for proper utilization of hospice benefits. Reporting allegations of patient abuse, neglect, and/or misappropriation of patient property. Following Standard Precautions and infection control procedures when in a clinical environment. Assuring safety needs of the patients are addressed and met. Assisting with on-call duties, referrals and admissions as needed. Demonstrating knowledge and competence in caring for hospice patients. Actively promoting an open line of communication and a professional relationship with patients/families, facility staff, all members of the hospice team and administration. Utilizing appropriate resources to resolve problems. Acting as a representative for the company in promoting hospice education in the community. Potentially traveling to multiple locations during a work day. Attendance is an essential job function. Complies with all local/state/federal regulations and standards of their profession. Complies with all company policies and procedures.
