Home Health Physical Therapist

We are seeking an experienced and compassionate Physical Therapist to join our Home Health team. In this role, you will assess, plan, and implement individualized therapy programs designed to improve mobility, relieve pain, restore function, and enhance patients' quality of life in the comfort of their homes.

Responsibilities

As a Physical Therapist, you will:

• Perform and document comprehensive initial evaluations to identify patient needs and establish appropriate treatment plans.
• Develop and implement individualized treatment programs to improve mobility, strength, balance, and overall functional independence.
• Regularly assess patient progress and modify treatment plans to achieve optimal outcomes.
• Educate patients and caregivers on therapeutic exercises, safety measures, and disease management strategies.
• Adhere to infection control standards and universal precautions during all patient interactions.
• Maintain timely, accurate, and complete clinical documentation in accordance with company policies and regulatory requirements.
• Collaborate effectively with physicians, nurses, caregivers, and interdisciplinary team members to ensure continuity of care.
• Communicate patient status, treatment progress, and recommendations to all appropriate parties.
• Promote and support the agency's mission, values, and clinical excellence standards.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current and unrestricted Physical Therapist license in the state of practice.
• Minimum of one (1) year of home health experience preferred.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• Current auto insurance.
• HomeCare HomeBase experience preferred.

Physical Requirements

• Ability to stand, walk, sit, bend, squat, kneel, and climb for extended periods.
• Ability to safely lift, carry, push, and pull up to 100 pounds with or without assistance.
