Registered Nurse (RN) – Case Manager

Tryfacta is seeking a Registered Nurse (RN) – Case Manager for our client in Eureka, CA 95501. This is a 13-week contract assignment. If you meet the qualifications listed below and are interested, please apply now!

Location: Eureka, CA 95501

Duration: 13 weeks

Work Schedule: Day 5x8-Hour (08:00 AM – 04:30 PM)

Responsibilities for this position include, but are not limited to:

• Coordinate patient care and discharge planning
• Perform utilization review and care coordination
• Assess patient needs and arrange DME/services
• Collaborate with physicians, social services, and interdisciplinary teams
• Ensure compliance with CMS, HIPAA, and Joint Commission standards
• Assist with admissions, discharge planning, and benefits coordination

To be considered for this position, you should have:

• 2+ years of Acute Hospital Case Management experience
• Current acute care Case Management experience within the last year
• Travel experience required (First-time travelers may be considered with manager approval)
• Epic charting experience required
• Active RN License
