Location: East Portland, OR and surrounding areas

Schedule: Day shift, 8-hour shifts (08:00 - 16:30), Monday through Friday. No weekend coverage required. May visit up to 6 patients per day with 1:1 patient ratios and 30 productivity points standard.

• Home health setting serving patients in East Portland and surrounding communities
\n
• Structured patient scheduling with minimal drive time between visits; occasional radius up to 60 miles per day
\n
• EPIC Remote Client for Home Health charting system with Rover phone app and Oasis documentation
\n
• Free parking and IRS standard mileage reimbursement provided
\n
• Business casual or scrubs allowed (no specific color requirement)
\n
• 91-day assignment with guaranteed hours (facility may call off up to one shift per two-week pay period)
\n

• Valid Oregon state license or compact license (must be in hand at time of submission)
\n
• Minimum 1 year of skilled home health experience required
\n
• Current BLS certification required
\n
• Reliable personal transportation required for patient visits
\n
• Experience with EPIC or similar charting systems preferred
\n
• Two professional references covering 1 year within the last 3 years
\n
