Responsibilities Include:

• Interact as a team member with our Interdisciplinary team

• Perform Nutrition Screens/Accurately assess nutrition risk levels

• Complete Nutrition Assessments-create appropriate goals and individualized care plans in efforts to achieve those goals

• Provide appropriate nutrition education

• May be required to travel to our different local facilities

• Ability to positively interact with our residents, staff and the public

• Be an active participant in problem solving and provide guidance in relation to Dining Services and Nutrition by following regulation standards

Qualifications Include:

• Candidates must have completed a certified Dietetic Technician program,

• Possess a title of Registered DTR or have a 4 year Nutrition Degree

• Food Service experience in long-term care setting preferred

• CMD & ServSafe certification preferred
