Hours per Shift: 8 (8:00 AM - 4:30 PM)

Experience: 2 years current strong Medical Surgical or Intensive Care Unit experience and Transplant experience preferred

License: Current Nursing License

Certifications: BLS

Must-Have: High level of skill in patient triage

Ability to triage simple and complex problems by telephone

Ability to use a computer keyboard to input and obtain data

Ability to document legibly using a writing instrument

Description: This position involves coordinating care and clinically managing post-transplant recipients. Responsibilities include prescription refills, clinics, triaging calls, and medication reconciliation. Reviewing charts and collaborating with other transplant disciplines such as pharmacy, social work, and nutrition are also required. Experience with heart transplant and EPIC is strongly preferred. The schedule will consist of three days per week with flexible days of the week. Nurses will be on site, in person, and will attend clinics.
