DESCRIPTION OF WORK Intake Support: Collect consumer information, document details, and explain available programs and eligibility Service Coordination: Arrange services for consumers based on simple or complex needs in cooperation with community resources Assessment Duties: Conduct evaluations in homes, facilities, or other settings to determine service needs and access to resources Case Management: Maintain caseloads, complete reassessments, and gather financial and personal information when required Communication Tasks: Prepare letters, reports, and records while responding to inquiries from families and professionals Record Maintenance: Track daily activities, update databases, and maintain confidential information with accuracy Work Schedule and Additional Information: Full-time employment Work hours are 8:00 AM to 4:30 PM, Monday - Friday, with 60-minute lunch. On-Call Hours When Required. Employee may have overnight travel or out of county travel for meetings or training. Telework: You will not have the option to telework in this position. REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY QUALIFICATIONS Minimum Experience and Training Requirements: Six months as an Aging Care Manager 1 or a County Caseworker 1; or Successful completion of the County Social Casework Intern program; or Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or An equivalent combination of experience and training.
