- Assist with the development and review of individualized recovery plans in collaboration with the program participant, clinical and medical staff, and other stakeholders identified with the participant.
- Help determine the resources and services needed to support each participant's individualized goals.
- Advocate for program participants to help ensure access to appropriate services and supports.
- Identify program needs and service gaps and recommend strategies for addressing them.
- Develop and maintain productive relationships with community agencies.
- Connect program participants with available community resources and services.
- Provide community-based services to program participants.
- Educate participants about available services, supports, and resources.
- Assist participants with making meaningful connections in their communities.
- Support participants in working toward the goals identified in their recovery plans.
- Transport program participants to appointments as required.
- Complete assessments, service documentation, and other records accurately and within required timeframes.
- Maintain documentation in accordance with RHD requirements and applicable funder, licensing, and accreditation standards.
- Participate in program planning, coordination, and service-review activities.

- Bachelor's degree in human services or a related field.
- At least two years of general work experience providing support to individuals receiving behavioral health, addiction recovery, or behavioral health home services.
- Intermediate computer skills.
- Valid driver's license.
- Ability to travel throughout the program's service-delivery area.
- Ability to transport program participants to appointments as required.
- At least one year of case management experience.
- This position provides services in community-based settings throughout the program's service-delivery area.
- Regular local travel is required.
- The Case Manager is required to drive program participants to appointments and other approved service-related destinations.
- Exerting up to 50 pounds of force occasionally.
- Exerting up to 20 pounds of force frequently.
- Exerting up to 10 pounds of force constantly to move objects.
- Standing or sitting for periods of time.
- Walking.
- Using hands and fingers to handle, operate, activate, prepare, inspect, or position objects.
