Key Responsibilities
- Evaluate patients and develop individualized physical therapy treatment plans within a skilled nursing facility setting
- Deliver hands-on therapeutic interventions to improve patient mobility, strength, and functional independence
- Monitor and document patient progress, adjusting treatment plans as clinically appropriate
- Collaborate with interdisciplinary care teams to ensure coordinated and comprehensive patient care
- Educate patients and their families on home exercise programs and safe mobility techniques
- Active and unrestricted Physical Therapist license in the state of Arizona
- Graduate of an accredited Physical Therapy program
- Experience working in a skilled nursing facility (SNF) setting
- Current CPR/BLS certification
- Prior travel or contract therapy experience
- Familiarity with SNF documentation and Medicare compliance requirements
- Opportunity to gain diverse clinical experience in a skilled nursing environment through a structured 13-week contract
