Responsibilities
- Provide direct patient care and nursing assessments within the LPN scope of practice, under supervision.
- Administer prescribed medications and treatments, monitor effects, and document administration accurately.
- Monitor vital signs and patient status; identify changes and promptly report to the supervising RN.
- Perform wound care, dressing changes, and routine procedures per the plan of care.
- Assist residents with activities of daily living (ADLs) and ensure safety, comfort, and dignity.
- Document care events, update care plans, and collaborate with the interdisciplinary team to support resident well-being.

Requirements
- Active Licensed Practical Nurse license in Vermont (or eligibility for licensure in the state).
- LPN diploma/certificate from an accredited program.
- Ability to work night shifts in a long-term care/skilled nursing setting.
- Strong communication, observation, and charting skills; reliable collaboration with care teams.
- Understanding of long-term care regulations and safety protocols.
