Responsibilities

• Assess and treat patients with musculoskeletal conditions to improve function.

• Develop personalized treatment plans and track patient progress over time.

• Document session notes and outcomes in the patient management system.

• Measure and report treatment outcomes to support performance-based incentives.

• Maintain on-site clinical readiness and ensure safe, compliant practice.

• Educate patients on exercises, posture, and home exercise programs.

• Collaborate with physicians and care teams to coordinate care.

• Utilize evidence-based practices to ensure high-quality care.

• Demonstrate reliability and accountability for personal productivity within a team.

• Identify at-risk patients and adjust care plans to prevent deterioration.

• Participate in ongoing professional development and skill advancement.

• Engage in pipeline-driven planning to identify growth opportunities within the territory.

Experience/Qualifications

• Licensed Physical Therapist with valid state license.

• Outpatient musculoskeletal PT experience with 2+ years preferred.

• Develop and document individualized treatment plans and progress notes.

• Demonstrated clinical reasoning and evidence-based practice.

• Proficient use of patient management system for notes and outcomes.

• Ability to measure and report treatment outcomes for incentives.

• Patient education on exercises, posture, and home programs.

• Collaborative communication with physicians and care teams.

• On-site clinical readiness, safety, and compliance with regulations.

• Track productivity and accountability within a team-driven environment.

• Identify at-risk patients and adapt care plans accordingly.

• Commitment to ongoing professional development and skill advancement.

• Pipeline-driven planning to identify growth opportunities within territory.
