Occupational Therapist (OT) | Mobile Outpatient Focused Care Rehab | McKinney, TX | Contract/W2 THE OPPORTUNITY Are you a OT who values meaningful patient relationships, professional independence, and the opportunity to see how therapy improves a patient’s daily life? Focused Care Rehab is seeking a licensed Occupational Therapist to provide one- on-one outpatient therapy to older adults in their homes and senior living communities throughout the McKinney area. You will treat one patient at a time in the environment where mobility matters most. Instead of managing several patients in a crowded clinic, you will help each patient work toward practical goals tied to safety, mobility, confidence, and independence. ABOUT FOCUSED CARE REHAB Focused Care Rehab provides physical, occupational, and speech therapy where patients live. Our patients receive care in private homes, assisted living communities, and other senior living settings. This allows our clinicians to address the mobility challenges patients face in their everyday environments, including walking safely, transferring, managing steps, reducing fall risk, and participating more fully in daily life. We believe strong care starts with listening, clear communication, thoughtful treatment, and respect for each patient’s goals. Our clinicians work independently while remaining connected to a team that values collaboration, responsiveness, and high professional standards. WHAT YOU’LL DO As a mobile OT, you will carry out treatment plans established by the supervising Occupational Therapist and help patients improve their strength, mobility, safety, and One-on-One Care: Provide skilled physical therapy interventions, including therapeutic exercise, gait training, balance activities, transfer training, and functional mobility. Real-World Treatment: Help patients address the specific challenges they face where they live, such as getting out of a chair, walking to the dining room, managing steps, or using an assistive device safely. Patient and Caregiver Education: Teach home exercise programs, safe mobility strategies, fall-prevention techniques, ADLs, and other recommendations established in the plan of care. Clinical Communication: Monitor patient progress and communicate changes in condition, concerns, and treatment outcomes to the director of rehab. Collaboration: Coordinate professionally with patients, families, caregivers, therapists, and senior living staff. Documentation: Complete accurate and timely documentation of interventions, patient response, and progress. Independence: Manage your schedule and travel between assigned patient locations while maintaining consistent communication with the clinical team. You Pay Attention: You notice changes in function, safety, symptoms, and patient response, and you raise concerns when appropriate. REQUIREMENTS Current Texas Occupational Therapist license Degree from an accredited Occupational Therapist program Valid driver’s license and reliable transportation Ability to travel between patient homes and senior living communities in the McKinney area Strong communication, organization, and time-management skills Ability to complete accurate documentation within required timeframes PREFERRED EXPERIENCE Working with older adults Outpatient or mobile outpatient therapy Senior living communities Orthopedic and musculoskeletal conditions Balance, gait, weakness, mobility, and fall-risk treatment -ADLs POSITION DETAILS Mileage or Travel Reimbursement: if outside of agreed upon region Primary Service Area: McKinney and surrounding areas (greatest need is at an Assisted Living and Memory Care facility in McKinney) WHY JOIN FOCUSED CARE REHAB?
