Work Schedule: Rotating 8–12 hour shifts averaging 40 hours per week

A nationally recognized military academic medical center is seeking experienced Clinical Pharmacists specializing in cardiology to support comprehensive inpatient cardiovascular care. Position Summary

The Clinical Pharmacist – Cardiology provides direct patient care through comprehensive medication management, cardiovascular pharmacotherapy optimization, anticoagulation management, medication reconciliation, clinical documentation, and interdisciplinary collaboration. The pharmacist serves as a medication expert supporting physicians, nursing staff, residents, and patients while promoting safe, effective, and cost-conscious medication use.

Key Responsibilities

• Provide comprehensive clinical pharmacy services to cardiology patients.
• Evaluate medication therapy daily and document recommendations within the medical record.
• Assess medication indication, dosing, allergies, interactions, duplication, laboratory monitoring, and therapeutic appropriateness.
• Manage anticoagulation therapy including initiation, monitoring, and adjustment of warfarin, direct oral anticoagulants, and heparin using established protocols.
• Optimize cardiovascular medication therapy.
• Perform medication therapy management and medication reconciliation.
• Monitor drug utilization, polypharmacy, and adverse medication events.
• Collaborate with physicians, nursing staff, and interdisciplinary care teams.
• Educate patients regarding medication adherence and anticoagulation safety.
• Provide pharmaceutical education to staff and pharmacy residents.
• Rotate to other clinical pharmacy assignments as trained and competent.
• Complete all required documentation in accordance with organizational policies.

Required Qualifications

• Doctor of Pharmacy (Pharm.D.).
• One of the following:
• ASHP-accredited PGY1 residency with Board of Pharmacy Specialties (BPS) certification.
• ASHP-accredited PGY2 residency.
• ASHP-accredited PGY1 residency plus three years of applicable clinical practice experience with Board of Pharmacy Specialties (BPS) certification.
• Current Board of Pharmacy Specialties (BPS) certification.
• Current unrestricted pharmacist license.
• American Heart Association ACLS certification.
• Recent inpatient hospital clinical experience within the past two years following graduation or residency.
• Ability to meet ASHP-qualified residency preceptor standards within one year of employment.

Preferred Qualifications

• Inpatient cardiology pharmacy experience.
• Experience in large academic or military medical centers.
• Strong interdisciplinary communication skills.
• Experience supporting pharmacy residency education.
