Hospice RN Case Manager

Hospice RN Case ManagerAs a Hospice RN Case Manager with Hospice Services of Massachusetts, you will be responsible for supervising hospice aides, completing comprehensive assessments of patient and family needs, and providing professional nursing care utilizing all elements of the nursing process. You will develop and implement individualized care plans, collaborate with interdisciplinary team members, and provide health care instructions to patients and families. Prior hospice experience is preferred, but not required.Full-time and part-time openings to cover Plymouth County and portions of Bristol County.
