Ambulatory Clinic Nurse Care Coordinator (RN) - Surgical and Urogynecology

This full-time Ambulatory Clinic RN Care Coordinator role focuses on providing patient‑centered care for individuals with pelvic floor disorders, urinary incontinence, prolapse, and postoperative recovery. Serves as a clinical liaison among patients, providers, staff, and community resources to ensure high‑quality care. Supports patient education, care planning, triage, and care navigation, balancing hands‑on nursing and back‑office duties, including nurse‑led procedures and provider assistance. Manages patient flow, coordinates care, and promotes self‑management, especially for high‑risk populations with multiple comorbidities. Utilizes electronic health records for documentation, communicates with patients and families, and ensures compliance with standards.
