Job Description:
- Actively monitor and enhance patient visit schedules to maximize efficiency and meet patient needs.
- Conduct comprehensive OASIS and PT assessments of patients, covering psychosocial, educational, safety, and risk factors in line with regulations and standards of care.
- Develop personalized plans of care aimed at promoting optimal functional independence and safety.
- Provide skilled physical therapy treatments, including therapeutic techniques and patient/caregiver education.
- Collaborate effectively with clinical team members, including physicians, nurses, caregivers, and rehab services, to ensure optimal patient care.
- Supervise and guide the clinical activities of PTAs, PT students, and home care aides.
- Ensure accurate completion of required paperwork in compliance with federal and state regulations and standards of care.
- Oversee billing processes to ensure accuracy and compliance with insurance coverage parameters.
- Engage in training programs for various stakeholders, including medical and nursing staff and community groups.
- Maintain and enhance personal knowledge in the field of physical therapy.
- Proactively promote Rehab services in community health fairs and other events.
- Perform patient-related health assessments and procedures as needed.
- Communicate timely changes in patient care schedules to schedulers and supervisors.
- Complete OASIS time points in accordance with regulatory guidelines.
- Cooperate with QI staff to ensure accuracy in clinical documentation.
- Follow agency protocols for McKesson processes to maintain visit integrity.
- Fill out agency-related forms efficiently and accurately.
- Conduct visits in a time-effective manner, ensuring supervisory visits are separate from routine assessments.
- Assess patients' needs for home care aide services and coordinate plans of care.
- Facilitate safe patient discharges and transfers, ensuring all paperwork is processed correctly.
- Submit incident reports to QI Specialists promptly when necessary.
- Actively participate in peer review audits and accept feedback professionally.
