Job specification

• Provide high-quality nursing care and education to Home Hemodialysis (HHD) patients, ensuring safe and effective treatment administration at home.
• Conduct thorough patient assessments, including physical, psychosocial, and lifestyle evaluations, to determine suitability for home dialysis.
• Develop individualized care plans and training programs to teach patients (and caregivers when applicable) how to operate dialysis equipment, follow safety protocols, and respond to complications.
• Monitor patient progress through home visits, telehealth check-ins, and collaboration with nephrologists and interdisciplinary renal care teams.
• Troubleshoot clinical or technical issues related to hemodialysis equipment and coordinate necessary maintenance or replacement.

Candidate requirements

• Bachelor of Science in Nursing (BSN) required; advanced nursing education or certifications in nephrology preferred.
• Current Registered Nurse (RN) license in the state of Indiana.
• Minimum of 1-2 years of hemodialysis experience; previous home dialysis or peritoneal dialysis experience strongly preferred.
• Current Basic Life Support (BLS) certification required; ACLS preferred.
• Strong teaching, assessment, and communication skills to effectively guide and support patients in home-based self-care.
