divh2Physical Therapist/h2pA Physical Therapist provides professional services within the scope of physical therapy practice standards in collaboration with primary care physicians and other members of the health care team and teaches and educates patients and their families./ppQualifications include successful completion and graduation from a PT Program approved by the Commission on Accreditation in PT education or an education program outside of the United States determined to be substantially equivalent to physical therapist entry level education in the US by a credentials eval organization approved by the American PT Association. A current, active license to practice Physical Therapy in CA is required. A minimum of two years experience as a Physical Therapist is also required. A criminal background check and current CPR certification are necessary. If trained outside of the United States before January 1, 2008, meets the following requirements:/pulliGraduated since 1928 from a PT curriculum approved in the country in which the curriculum was located and in which there is a member organization of the World Conference for Physical Therapy./liliMeets the requirements for membership in a membership organization of the World Confederation for Physical Therapy./li/ulpResponsibilities/Essential Functions include receiving the intake referral information and preparing PT paperwork/tools necessary for the visit. Obtaining all pertinent medical history from patient, family or significant others. Performing PT assessment visit and documenting accordingly. Performing home safety check and environmental assessment of the patients home environment. Assisting the physician in evaluating the level of function. Helping develop an appropriate and effective PT Plan of Care to be submitted to the physician for approval and implementation. Functionally reassess the patient for on-going therapy. Determining medical necessity for other services. Evaluating the patients ADL and iADL abilities. Developing and implementing the HHA plan of care when HHA services are ordered and PT is the only skilled discipline. Supervising the HHA in accordance with state/federal requirements and documenting the supervision without having to be directed to do so, when PT is the only skilled discipline. Effectively communicating ongoing with patient and family regarding the POC and progression of such. Instructing patients and families in the safe use of prosthetic, orthotic, and assistive devices. Communicating with other disciplines in the case. Communicating with the patients physician to obtain effective treatment modalities and/or rehabilitative therapy modalities. Assisting the physician in evaluating patients by applying diagnostic and prognostic muscle, nerve, joint and functional ability tests. Submitting ALL OASIS assessments/documentation within 48 hours of the OASIS visit. Performing clinical record reviews in collaboration with the Nursing Supervisor. Participating in the Performance Improvement Committee process as requested. Participating in staff meetings and in-services. Participating in the growth of the office by being a willing preceptor for newly hired, same discipline employees. Participating in the planning, operation and evaluation of the therapy services of the organization. Maintaining professional licensure and actively seeking out educational experiences to enhance the practice of home care physical therapy and the benefit of the patients serviced. Notifying the Agency of emergencies, sickness, and other imminent occurrences that may affect the patient caseload as quickly as possible relative to the events occurrence. Submitting written time requests 2 weeks or more in advance of planned time off. Other duties as assigned./ppWork Environment includes indoors, performing patient assessment/tasks in the patients residence. Travel to patient residences/meetings; will require transportation to get to various work sites. Possible exposure to communicable diseases./ppFunctional Abilities include being able to access patients homes that may not be handicap accessible. Being able to hear, speak, understand and communicate effectively in English./ppPhysical Requirements include on a daily basis: walking, standing, sitting, reaching, use of telephone, use of computers other office equipment, lifting/transferring 150 lbs or greater, bending, stooping, climbing stairs./p/div
