Join Our Caring CommunityAs a Patient Care Coordinator, you will:Work as part of our local sales team and report to a Market Executive of SalesCoordinate care from hospitals and SNFs in the territoryMeet with the patients at the bedside to discuss home health services, disciplines and what to expect at homeSupport operations teams by ensuring they have the resources, information, and coordination needed to achieve a successful start of careAssist with sales calls as needed to support Account Executive in addressing client needs and advancing business objectivesUse Your Skills to Make an ImpactRequired/Preferred Experience/Skills:LPN, LVN or RN license is requiredKnowledge of governmental regulations, Medicare eligibility requirements, comprehensive understanding of potential care plan needs for the patient and coordination of necessary resourcesThree years clinical experience preferredExperience with EMR systems such as Watershed and Olio highly preferredTravel within assigned geographical territory is requiredThis role is considered patient facing and is part of Human's Tuberculosis (TB) screening program.
