Provide direct patient care, evaluate outcomes, and collaborate with interdisciplinary teams and specialists to adjust care plans and ensure optimal results. Coordinate care with patients, families, providers, and community resources to support continuity and holistic care. Educate patients and families on care plans, treatment needs, patient rights, and informed consent, using effective teaching methods and tools. Maintain current knowledge of nursing best practices, research, standards, and organizational policies to support high-quality care. Demonstrate proficiency in clinical equipment and support safe, effective use in patient care. Promote strong team collaboration through clear communication, problem-solving, and conflict resolution. Perform on-call responsibilities, including triaging patient needs and providing in-home visits as required. Pursue ongoing professional development to maintain expertise in key clinical areas. Perform other duties as assigned. Qualifications Graduate of an accredited nursing program (minimum Associate Degree in Nursing required).
