Manager Nurse RN

Community: Newcastle Place Address: 12600 N Port Mequon, Wisconsin 53092 Pay Range $89,500.00-$123,100.00+ Annual Nurse Manager, RN for Assisted Living and Memory Care. Responsibilities Review resident referrals and respond to home care service requests and inquiries. Coordinate completion of resident assessments and develop care plans, collaborating with physicians and other health care professionals to implement and maintain the care plan. Market home health services to residents. Promote home care and wellness programs by conducting resident clinics and sponsoring resident trainings. Monitor resident changes and update care plans and service contracts. Contact physicians and family members as necessary. Qualifications Registered Nurse licensure as required by state regulation.
