Description

• Conduct thorough evaluations of patients in various settings, both inpatient and outpatient, creating tailored treatment strategies
• Implement physical therapy interventions targeting mobility, strength, balance, and overall functional improvement
• Work closely with diverse healthcare professionals, including nurses, physicians, case managers, and rehabilitation staff, to enhance patient outcomes
• Engage in patient and family education regarding therapeutic objectives, discharge processes, and recommended home exercises
• Maintain accurate and comprehensive documentation as per facility protocols and regulatory requirements
• Contribute to interdisciplinary meetings and care rounds to coordinate patient care effectively

Education

• Doctorate in Physical Therapy (DPT) or a relevant advanced degree from an accredited institution

Certifications

• Current state licensure as a Physical Therapist
• CPR certification preferred
• Additional specialized certifications (e.g., neurological, geriatric) are welcome but not mandatory

Skills

• Prior experience is highly preferred
