Community Outreach Manager

The Community Outreach Manager is responsible for overall leadership, development, and oversite of a team of Community Outreach Advocates. The main purpose of the Community Outreach Advocate team is to educate and enroll new providers while increasing the number of patients that benefit from their no cost Medication Assistance Program to those that qualify. It’s a rewarding role that can make a significant impact for those in need across the state of South Carolina. Serves as the direct report for the advocates and provides guidance and support to ensure that our team of advocates can effectively serve as resources to patients and partnering facilities and community organizations for Medication Assistance Program. Responsibilities Providing training and support to the Advocacy team and identifying any learning and ongoing training needs. Maintains current knowledge of the Medication Assistance Program eligibility requirements, policies, formulary, and application process. Coordinates with Director to review and maintain monthly reporting on advocate activities, progress, updates, and follow-up within their designated region. Works to create and build trusting relationships and referrals with the Department of Corrections (DOC) across the state through direct training and education with the purpose of the DOC utilizing their program as a resource to people being released that need medication assistance. Analyzes and reports data needed to evaluate and assess target areas within communities to increase knowledge and utilization of the Medication Assistance Program Identifying and implementing both short and long-range goals and methods to increase community accessibility and participation to the medication assistance program. Research and pursue new resources and advocacy opportunities through newsletters, events, networking, and current state data. Facilitating monthly meetings with each advocate to stay in communication and discuss needs, concerns, and progress both individually and within their region. Tracks outcomes including patients served, pending applications and follow-up, as well as changes in number of approvals and works to create an action plan with advocates where needed. Complete performance evaluations for assigned staff Requirements Bachelor's degree (preferred) or high school diploma and at least 3 years of leadership and experience that is directly related to the duties and responsibilities specified; OR at least 7 years of experience that can be demonstrated to be applicable to the duties listed in the job description.
