As a Home Health LPN, you'll provide one-on-one patient care, support recovery, educate patients and families, and work alongside a collaborative clinical team focused on positive patient outcomes.

What You'll Do

• Provide skilled nursing care in patients' homes under physician orders
• Follow care plans and goals created by the RN Case Manager
• Monitor patient conditions and report changes
• Administer medications and treatments as prescribed
• Educate patients and families on disease management and preventative care
• Help reduce re-hospitalizations through patient education and proactive care
• Complete clinical documentation in accordance with agency and Medicare guidelines
• Participate in on-call, case conferences, trainings, and required meetings

Qualifications

• Current Licensed Practical Nurse license in the state of employment
• Minimum of 1 year of LPN experience in an acute care setting required
• 2+ years preferred
• Home health experience preferred
• Valid driver's license and active auto insurance
• Strong communication, time management, and documentation skills
• Compassionate, dependable, and patient-focused
