Job Summary

We are seeking a dedicated and skilled Registered Nurse (RN) Team Lead for a 13-week contract assignment in a 120-bed Skilled Nursing Facility (SNF) located in Montour Falls, New York. This facility is committed to delivering high-quality healthcare in partnership with the community, focusing on personalized patient care. The facility is structured into three units, each accommodating a maximum of 40 residents and overseen by an RN Manager.

The RN Team Lead will play a pivotal role in coordinating patient care, demonstrating strong leadership in time management, critical thinking, delegation, and prioritization. This position involves direct patient care, including assessment, monitoring, recording, and reporting patient conditions, while collaborating closely with the interdisciplinary care team. The successful candidate will advocate for patients and ensure ethical, evidence-based practices are maintained. Flexibility to perform duties within the scope of an LPN Med Cart position may be required based on staffing needs.

Key Responsibilities

• Provide direct patient care, including comprehensive assessment, monitoring, recording, and reporting of patient conditions.
• Demonstrate strong time management, critical thinking, delegation, and prioritization skills in a team lead capacity.
• Collaborate effectively with the interdisciplinary care team to coordinate and optimize patient care plans.
• Act as a patient advocate, ensuring ethical and professional practice within the scope of nursing.
• Promote evidence-based practice (EBP) and best practices related to patient care delivery.
• Maintain clinical competence specific to the care area, including completion of all required organizational education within stated timeframes.
• Communicate effectively with patients, families, and the healthcare team.
• Document patient care accurately and thoroughly in the Electronic Medical Record (EMR).
• Participate in unit councils and staff meetings to contribute to unit improvements and team collaboration.
• Float to other units as staffing permits to support overall facility needs.

Required Qualifications

• Active Registered Nurse (RN) license in New York State.
• Ability to perform duties within the scope of a Licensed Practical Nurse (LPN) as staffing needs dictate.
• Physical ability to lift up to 50 lbs.
• Ability to sit or stand for long periods as required by job duties.
• Must be able to physically operate the equipment required for the job.
