Licensed Clinical Social Worker (LCSW) – Ft. Thomas 20 North Grand Ave, Fort Thomas, Kentucky Job Type Regular Scheduled Hours 40 hours per week Job Summary Reports to the Social Work Manager, the Licensed Clinical Social Worker (LCSW) is a member of the Social Work Department located in the primary care offices to provide care management services to patients. The LCSW conducts face‑to‑face comprehensive biopsychosocial assessments, collaborates with patients’ primary care physicians to determine diagnosis and treatment, administers brief therapy, and follows patients throughout the therapy process. Work is with adult and geriatric patients presenting with emotional disorders and mental illness in a primary care setting. The LCSW demonstrates respect, dignity, kindness, and empathy toward all patients, families, visitors, and colleagues regardless of cultural background. Duties & Responsibilities Provide clinical social work: assess, collaborate with primary care physicians, diagnose, and treat emotional and mental illness related to overall health. Conduct comprehensive psychosocial assessments, identify bio‑psychosocial and environmental factors to establish a care plan. Initiate contact with patient/family within one business day of referral. Document and communicate information on chart in accordance with HIPAA guidelines. Establish a positive therapeutic relationship to correct dysfunction. Use evidence‑based treatment modalities (CBT, solution‑focused, motivational interviewing, crisis intervention, mindfulness, trauma‑informed, behavior activation, acceptance and commitment, person‑centered, etc.) to stabilize and regulate dysfunction. Identify social determinants and barriers, and refer patients for case management as needed. Develop, implement, and modify treatment plans. Evaluate progress and adjust care plans. Terminate treatment or refer to higher level of care when necessary. Initiate contact with primary care patients discharged from St. Elizabeth emergency room after a mental health evaluation. Document all visits and phone contacts accurately and timely; record interventions and case closure plans in EPIC. Provide patient/family education, counseling, and social work interventions to enhance problem solving and coping. Serve as liaison between patient/family and healthcare team. Continuously seek to improve quality and value of services to patients, families, and health teams. Build rapport with patients, families, and healthcare teams. Expand and strengthen patient’s social supports. Consult with interdisciplinary team on psychosocial components to promote understanding of patient. Communicate effectively with co‑workers and healthcare team to ensure continuity of care. Address employee care matters with behavior standards; resolve issues directly, respectfully, and constructively. Maintain competence in the medical social work role and pursue continuing education to fulfill licensure requirements. Assist in evaluating patient outcomes and departmental performance.
