Position Summary: The Dietitian will provide nutritional assessments, dietary counseling, education and interventions to the terminally ill patient and their family and/or care givers. The Dietitian is an active participant in the Agency's Interdisciplinary Group (IDG) and assists in developing, coordinating, and implementing an individualized Plan of Care (POC). Qualifications: Bachelor’s Degree in nutrition or related subjects, preferred. Registered or Licensed Dietitian as required by the state where practicing; or is eligible for registration by the American Dietetic Association or has documented equivalency in education or training. Must have current driver’s license. One (1) year experience as a dietician/dietitian. Experience in hospice or working with terminally ill patients preferred. Knowledge of nutritional needs of the terminally ill patient. Good interpersonal skills. Demonstrates ability to work with electronic medical records. Good understanding of hospice philosophy. Reliable transportation and valid current auto liability insurance required.
