How you'll make an impact in this role

• Reduce Readmissions & Ensure Safety: Oversee complex discharge plans, ensuring patients have crucial resources at home, such as housing, follow-up care, and medications.
• They tackle non-medical issues and address the root causes: referred to as Social Determinants of Health (SDOH)-like food insecurity, housing instability, and financial challenges that affect patients' ability to heal and adhere to treatment
• Provide Emotional Support: Deliver vital mental health counseling, offer crisis intervention, and connect patients to behavioral health services.
• Provide advocacy & resource expertise: Serve as a strong advocate for patients and families, coordinating various community resources to improve the patient's overall well-being post-discharge.

What minimum requirements you'll need

Licensure / Certification / Registration:

• Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. ACM also accepted.
• Certified Social Worker preferred.

Education:

• Bachelor's degree in Social Work required.

Work Experience:

• 3 years of experience preferred.

What additional preferences we're seeking

• Masters Degree in Social Work; LCSW or LMSW
• Hospital Discharge Planning Experience preferred; Clinical Social Work experience strongly preferred
• Supervision available
