Care Coordinator, Care Anywhere (Remote AZ, CA, NV | Bilingual Spanish | Medical Assistant)

We invite you to join our Care Anywhere team as a Remote bilingual Spanish Care Coordinator. This high‑impact member‑facing role is open to candidates located in Arizona, California, or Nevada and aligns with Pacific Time business hours. The role focuses on outbound outreach (75‑80% of the time) to engage members, schedule visits, and maintain ongoing relationships. Schedule Monday–Friday Option1: 8:00AM–5:00PM PT (1‑hour lunch) Option2: 8:30AM–5:30PM PT (30‑minute lunch) Responsibilities Manage (4) provider schedules to ensure schedules are filled. Prepare charts for upcoming home visit appointments (check member eligibility, gather records needed by the provider prior to the home visit). Conduct outreach for scheduling, appointment confirmation calls, wellness checks for high‑risk members, and connect with providers/pharmacies to address member needs. Handle inbound/outbound calls (60–80 calls/day). Obtain medical records from provider offices, hospitals, and skilled nursing facilities (SNF) and upload to EMR. Submit referral authorizations to independent physician association (IPA)/medical groups for specialty, durable medical equipment (DME), and home health (HH) services. Coordinate lab orders, transportation for high‑risk members. Document calls in EMR. Support short‑message service (SMS) and member outreach campaigns. Assist nurse practitioner (NP) team with visit preparation needs. Remind members of appointments. Assign members to NP in EHR. Provide needed documentation to NP for visits each day. Direct inbound calls regarding medication refills. Maintain and update members' records. Mail/fax correspondence to primary care physicians (PCP), specialists, as needed. Attend Care Anywhere meetings / presentations. Recognize work‑related problems and contribute solutions. Work with outside vendors to provide appropriate care needs for members. Job Requirements Experience: Minimum 1 year entering referrals and prior authorizations in a healthcare setting; preferred 2 years. Education: High School Diploma or GED required; preferred completion of medical assistant program from an accredited school. Training / Certifications: Preferred medical terminology and specialized skills; knowledge of ICD9 and CPT codes, managed care plans; 10‑key typing at least 40 WPM; proficiency with Microsoft Office suite; effective written and verbal communication skills. Language Skills: Bilingual English/Spanish required; preferred. Technical Skills: Knowledge of Athena, ability to use EMR system. Licensure: Required none; preferred medical assistant certificate, medical terminology certificate. Physical & Other Skills: Basic math ability (addition, subtraction, multiplication, division, handling money and measurements); reasoning to apply common sense; effective problem solving, organizational and time‑management skills.
