Responsibilities and Expectations: Lead and collaborate in the management and delivery of patient care plans. Provide case management and coordinate home health services. Collaborate with interdisciplinary teams to ensure quality patient care. One year of RN experience and clinical competence to deliver quality patient care. Current driver’s license and ability to spend ~20% of your day driving to/from patient locations. Commitment to consistently meet critical deadlines for charting. Strong time and schedule self‑management skills. Demonstrated experience with tablets, mobile phones and EMR software.
