Doctorate in Physical Therapy (DPT) or a related degree from an accredited institution

State licensure as a Physical Therapist

CPR certification is a plus

Specialized certifications (neurological, geriatric, etc.) are advantageous but not mandatory

Prior experience is highly desirable
