Work independently in the community and from your home office Meaningful work that directly impacts individuals and families Ongoing assessment opportunities in your local area Administrative support from an experienced team Long-term contract opportunity with consistent work available What You'll Do Conduct in-person Medicaid assessments using the SIS-A assessment tool Meet with individuals, caregivers, and support teams in community settings Evaluate functional needs and document assessment findings accurately Complete and submit reports electronically within required timeframes Travel throughout Clinton and Lycoming Counties as needed Maintain professional communication while providing compassionate, person-centered service Qualifications Active Registered Nurse (RN) license Experience working with individuals with Intellectual and Developmental Disabilities (IDD), Autism, behavioral health, long-term services and supports, or case management preferred Strong assessment, documentation, and communication skills Ability to work independently and manage your own schedule Reliable transportation and willingness to travel within the assigned counties Comfortable using technology to complete electronic documentation Preferred Experience Candidates with experience in any of the following are encouraged to apply: Medicaid assessments Intellectual & Developmental Disabilities (IDD) Autism services Case management Community health Behavioral health Long-term services and supports (LTSS) Home and community-based services (HCBS) Care coordination Apply Today We're actively expanding our assessment team in Pennsylvania and have a limited number of contractor opportunities available.
