RN - Home Health Profession: Registered Nurse Specialty: Home Health Duration: 13 weeks Shift: Day 5x8-Hour Hours per Shift: 08:00 - 17:00 Experience: Minimum 2 years required License: RN license in California Certifications: BLS Must-Have: Ability to float to various areas Experience with Epic charting system On-call availability for 1 day per month Ability to work every other weekend Description:This position involves performing in-home nursing care, including direct patient care and patient/family education.The nurse will assess, coordinate, and evaluate in-home nursing care.Holistic and individualized care are essential for all patients.The nurse will develop and implement an interdisciplinary plan of care.This plan must consider psychological, sociocultural, spiritual, economic, and lifestyle factors.The role requires fostering collaborative relationships among patients, families, physicians, and other healthcare providers.Effective communication is vital in this position.The nurse will adhere to policies, industry standards, and applicable laws to promote a quality patient experience.Engagement in continuous growth and development in professional nursing practice is expected.
