Job Title: RN, Care Manager (Home Health) Profession: RN Specialty: Care Manager (Home Health) Duration: 13 weeks Shift: Day Shift Hours per Shift: 8 Experience: Minimum years of experience required in home care or acute care facility preferred License: Registered Nurse (RN) licensure in the state of practice required Certifications: CPR or BLS certification from an approved vendor required Must-Have: Valid Driver's License and must be at least 21 years of age or older required; current automobile insurance and a reliable automobile required Description: Our home health and hospice agencies serve the entire community and offer personalized in-home care. Job Summary: Plans, organizes, and directs home care services to meet the needs of patients in their homes. Manages a patient caseload effectively and efficiently. Provides patient care independently while collaborating with other core disciplines according to the patients' plan of care. Functions as a resource to patients, family members, and other care team members. Job Requirements: Education and Work Experience: Bachelor's Degree in Nursing (BSN) preferred. Acute care facility or home care experience preferred. Essential Functions: Completes initial and ongoing comprehensive assessments of patient needs at appropriate time points. Completes the nursing assessment of assigned patients. Discusses and teaches pertinent aspects of nursing care and patient rights and responsibilities. Identifies and reports problems, abnormal findings, or nursing diagnoses needing follow-up. Implements the plan of care in accordance with identified needs. Participates with the patient in clarifying mutually agreed-upon goals and incorporates these into the plan of care. Provides direct and/or indirect skilled, safe, and quality care. Coordinates patient care with other disciplines and integrates input from various disciplines. Performs ongoing nursing assessments and modifies plans of care as needed. Accurately documents all pertinent information in accordance with established policies and procedures. Ensures that documentation is completed and submitted in a timely manner. Updates nursing plans and medication profiles when changes occur. Counsels and educates patients and families in meeting healthcare and nursing needs. Provides learning opportunities for patients and caregivers, including written material as appropriate. Initiates appropriate preventative and rehabilitative nursing procedures. Complies with all applicable legal requirements, standards, policies and procedures. Demonstrates professional conduct and a commitment to service excellence. On-call duties may be assigned based on operational needs. Performs other job-related duties as assigned.
