Position Care Manager I / CCBHC Care Coordinator II Location Sullivan, IN Job ID 2583-3613 # of Openings 1 Summary The Care Manager I / CCBHC Care Coordinator II will provide case coordination, outreach, and advocacy services to individuals entering CCBHC. They will provide initial screening and referral services, assist in developing psychosocial evaluation, conduct initial health screenings, and develop a coordinated treatment plan. The role involves delivering comprehensive services for children and adults including 24/7 crisis services, outpatient mental health and substance abuse treatment, risk assessments, diagnoses, and care coordination with emergency rooms, law enforcement, and veteran groups. Essential Duties / Responsibilities Consistently exercise discretion and judgment to analyze, interpret, make deductions, and decide what actions are necessary based on the varying facts and circumstances of each case. Works without daily supervision, evaluating possible courses of conduct and making decisions independently. Maintain an active caseload providing referrals and linkages as needed within areas of expertise and credentials limits. Aquire and provide the team with detailed information regarding an assigned consumer to establish the foundation for the Treatment Plan. Work with the consumer on a day‑to‑day basis using professional judgment to implement the team‑determined Treatment Plan. Assist in development, implementation, and revision of individual treatment plans; ensure services specified in the Treatment Plan are delivered and monitor progress toward goals. Communicate consistently with community partners to maintain positive working relationships for existing MOUs. Consult and coordinate with community systems to facilitate linkage, referral, crisis management, advocacy, and follow‑up focusing on attaining treatment goals. Provide crisis management for consumers; make linkages for interventions as appropriate. Provide individual and group psychosocial rehabilitative services. Provide face‑to‑face skill training and mental health interventions in accordance with the treatment plan. Provide age‑appropriate life skills training individually or in groups. Maintain consumer and program records according to applicable standards, regulations, and grant requirements. Maintain a high level of ethical conduct regarding confidentiality, dual relationships, and professional stature. Demonstrate competent use of Electronic Medical Record (EMR). Participate in continuing education activities, remaining knowledgeable in the area(s) of expertise. Attend meetings and meet regularly with supervisor to exchange pertinent information and receive supervision. Complete CANS/ANSA training and ongoing certification. Complete DARMHA Supported Consumer paperwork and reassessments within specified deadlines. OBHP qualified staff participate in required clinical supervision. Maintain a flexible schedule in collaboration with supervisor to meet the needs of consumers and team. Perform other duties as assigned. Minimum Qualifications – Care Manager I High School diploma or equivalent. At least 12 months paid full‑time or 24 months paid part‑time employment in a related adult, child, youth, adolescent, or family services field (e.g., community mental health center, group home, residential placement facilities, caregiver, state hospital, nursing assistant, licensed professional nurse, etc.). Minimum Qualifications – CCBHC Care Coordinator II Bachelor’s degree in psychology, social work, sociology, family/consumer sciences, child development, substance abuse, mental health, human services, education, criminology, counseling, nursing, special education, or related field. Minimum of 1 year of case management/care management experience (internal or external). Completion of case management training post‑college degree. All Positions Must Possess Knowledge and skills in community‑based behavioral health care and case management. Good interpersonal and communication skills. Agreement to use personal transportation for work including transporting consumers. Willingness to use reliable personal transportation. Certificates / Licenses / Registrations Valid driver’s license in accordance with HCI motor vehicle policy. Maintain current Crisis Prevention Intervention (CPI) certification. Maintain current American Red Cross CPR/First Aid certification. Physical Demands Employee regularly communicates in person or by telephone, travels to consumer locations, stands, walks, reaches, bends, uses hands to handle objects, and occasionally lifts/moves up to 40 pounds. Occasional sitting is required. Work Environment Overtime rate after 40 working hours. On‑call schedule in support of crisis. Nights and weekends periodically. Conditions of Employment Satisfactory reference and background investigation checks. Pre‑employment drug screening and post‑employment drug or alcohol tests upon reasonable suspicion. Completed background checks at hire and every four years as required for direct contact with DCS consumers. Fingerprints required. Computer literacy demonstrated through pre‑employment testing. Completed tuberculin screening and annual Hep B, influenza and coronavirus vaccinations. Completed Center‑wide orientation and all required paperwork before reporting to work. Completion of Recovery Works Training, DCS Training, HCI Quality Training, Trauma‑Informed Care, e‑Learning, CPI Training, CPR/First Aid Training, annual restraint training (for IPU staff), HCI OBHP training modules, mandatory staff development and training, EMR orientation, Initial Competency Assessment, New Employee Department Checklist, six‑month orientation, Annual PES, and adherence to all HCI policies, payroll deposit, and compliance program.
