Minimum Requirements

Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
Active Michigan Physical Therapist license required to start the assignment.
BLS (Basic Life Support) certification required from AHA or ARC.
Home Health experience required. Candidate must be certified or have demonstrated competency working in home health settings.
Experience with EPIC electronic health record preferred.
Ability to complete required documentation and travel to patient residences within assigned service area.
Preferred: prior travel or agency experience and experience managing caseloads in community or home-based care.

Assignment Details

Facility Type: Home Health.
Shift Name: Days.
Shift Details: Monday - Friday, 08:00 - 17:00.
Holiday rotation: no weekends or holidays.
Start: 04/06/2026. Assignment length: 12 week.
