Physical Therapist (PRN)

Role Summary: The Physical Therapist provides age-appropriate hospice services, including assessments, direct treatment, development of home exercise programs, patient and family education, adaptive equipment evaluation, and thorough documentation in line with hospice standards. Key Responsibilities Conducts physical therapy evaluations based on physician orders and Plan of Care for symptom management and maintaining daily functional skills. Documents patient visits, reports reactions to treatment, and updates physicians and administrators through the Care Management process. Participates in the Interdisciplinary Group, offering input for the patient's Plan of Care. Instructs patients, families, and hospice team members on therapeutic activities and progress toward goals. Prepares discharge summaries when therapy services end, ensuring records are updated and shared appropriately. Provides guidance to patients and families on physical therapy techniques as needed. Qualifications & Skills Current Physical Therapy licensure in the practicing state. Current CPR certification. Ability to work effectively in diverse home environments. Valid driver’s license, vehicle insurance, and access to transportation. Preferred: Strong multitasking, independence, communication, organizational skills.
