How You'll Make An Impact In This Role

• Lead the implementation of personalized rehabilitation care plans, empowering patients to reach their highest level of physical and cognitive independence.
• Execute physician and nursing orders with precision to ensure clinical interventions are both safe and effective for long-term recovery.
• Monitor and document patient progress with a keen eye for detail, communicating vital status changes to the care team to ensure timely adjustments in treatment.
• Facilitate comprehensive discharge planning alongside a multidisciplinary team to ensure patients and their families have a safe, confident transition back to their community.

What Minimum Requirements You'll Need

Licensure / Certification / Registration:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required.

Education:

• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required.

What Additional Preferences We're Seeking

Prior nursing experience preferred to be successful in this role.
