Provide conscientious commitment to assigned work, schedule, honesty and teamwork. Recognize and manage risks, resolve issues and problems promptly, report and document adverse events and reportable conditions, and support Memorial Healthcare System's mission and values verbally and behaviorally. Provide social work services to patients, family members and significant others in four areas: 1) psychological functioning, 2) adaptation to illness, injury, disability or life event, 3) psychosocial assessment/evaluation and 4) facilitation services such as referral to community agencies, financial planning, and provision of information and resource knowledge. Identify and intervene for suspected abuse, including age‑specific concerns: physical/emotional/sexual abuse of children and adolescents, domestic abuse for adults, and caretaker/institutional abuse for senior adults. Manage crisis situations by quickly appraising the situation and selecting appropriate intervention. Document direct service in the patient’s medical record after each significant encounter. Demonstrate proficiency in using the electronic medical record and show good problem‑solving skills, root‑cause analysis, and principles of clinical performance improvement. Required Work Experience: One (1) year of social work experience in a health‑care setting.
