What You'll Do

• Provide nursing care, education, and supervision for patients in their homes.
• Assess, plan, and implement nursing interventions to promote, maintain, or restore health.
• Collaborate with healthcare teams and community partners to coordinate patient care.
• Document patient responses and interventions accurately.
• Evaluate patient progress and adjust care plans accordingly.
• Exercise independent judgment in emergency and routine situations.
• Maintain confidentiality and adhere to all regulatory standards.
• Be flexible with work hours and accept on-call responsibilities.

What You Need

• Valid Massachusetts RN license
• Ability to exercise independent judgment and demonstrate interpersonal skills
• Critical thinking and emotional stability in emergency settings
• Valid driver's license and ability to drive in various weather conditions
• Willingness to accept on-call duties and limited time-off requests

Preferred:

• Experience in community health nursing
• Ability to supervise others and develop patient care records
