POSITION SUMMARY - RN Case Manager


POSITION DUTIES - Coordinate patient care from admission through discharge, ensuring that treatments, services, and resources align with each patient's medical needs and recovery goals. Collaborate closely with physicians, nurses, and external providers to remove barriers to care, streamline transitions, and support safe, efficient discharge planning.


MINIMUM REQUIRED QUALIFICATIONS -

• CA RN license
• BLS
• Strong acute care hospital experience.
• 3+ years of inpatient, acute care hospital experience and/or acute care case management.
• Utilization review & discharge planning experience.
• Previous Dignity/CommonSpirit experience in specialty.

PREFERRED QUALIFICATIONS - Previous travel experience


LENGTH OF ASSIGNMENT - 13 Weeks


SHIFT / HOURS PER WEEK - 800A-4:30P (approx) with every other weekend


SYSTEMS - Cerner


START DATE - ASAP
